I’ve never seen such a blatant scam.
It asks for every single bit of credit card info, plus all your personal data for the full identity theft package.
Unfortunately I entered the card number in the wrong format. It then redirected me to a naughty website. I wonder if that was on purpose to deter scambaiters or trolls or if anyone who sends data gets there.
